Which Medicare markets deserve attention?

Across 3,199 counties, MA & other health plans cover 51.6% of the beneficiaries in the county records (county-summed; the direct CMS national record is 51.1%, June 2026) — but the county spread runs from under 5% to over 90%. 1,454 counties are already majority-MA, and 2,247 of 3,195 gained share in the last twelve months. Data through June 2026 · CMS Medicare Monthly Enrollment · ingested September 26, 2026 Verified Year-over-year vs June 2025

MA & other health plan share is the share of a county's Medicare beneficiaries enrolled in Medicare Advantage or another Medicare health plan (cost plans, PACE and demonstrations included), rather than Original Medicare. A county at 70% means seven in ten local Medicare beneficiaries are enrolled in Medicare Advantage or another Medicare health plan rather than Original Medicare.

Basis: MA & other health plans ÷ all Medicare beneficiaries (Parts A and/or B). The CMS numerator MA_AND_OTH_BENES counts Medicare Advantage plus cost plans, PACE and demonstrations, so this is not an MA-only share.

Share change is that share today minus the same month a year ago, in percentage points. Unlike enrollment growth, it is not raised by growth in the Medicare population itself.

Two counties with the same share can have moved differently: a 70% county that gained 0.2 points and a 40% county that gained 3 points are different situations. Neither figure shows why the share moved, or where future enrollment will come from; the charts below plot both so the question can be investigated.

Each ball is one county. Further right means a higher MA & other health plan share; higher up means the share rose more over 12 months; further back means more MA & other health plan enrollees.

The left, high and far back corner holds large counties where the share is lower but rose over the year. That pattern records recent share gains in a large county; it does not show that more beneficiaries will switch, or that the county is an opportunity for a particular plan. The right and flat corner is the opposite. Investigate In a large county with a high share and little change, how much movement is between plans rather than out of Original Medicare? These counts cannot say; plan-level enrollment (CMS CPSC) and your own disenrollment data can.

Where this comes from

Every figure on this page is computed at build time from the CMS Medicare Monthly Enrollment file, county geography level (BENE_GEO_LVL=County), June 2026, compared against June 2025 from the same file.

MA & other health plan share here uses the monthly-file basis: MA_AND_OTH_BENES ÷ TOT_BENES. That numerator includes cost plans, PACE and demonstrations alongside Medicare Advantage, so it is not an MA-only share. The bases are documented at Methodology → share bases.

† Small base: the county has fewer than 1,000 Medicare beneficiaries, so a few members move its share and 12-month change by whole points. Read those changes with care.

CMS suppresses cells with fewer than 11 beneficiaries; those counties are shown with an em dash rather than a zero. Summed, the county rows hold 69,999,135 beneficiaries, 609,922 fewer than the CMS National record (70,609,057), so the county-summed share (51.6%) is a reconciliation figure, not the national share (51.1%); what the difference is made of.

Counties covered: 3,199 · with a year-ago comparison: 3,195 · plotted on the map: 3,107 (Puerto Rico and the Virgin Islands appear in the table and CSV but not on the continental map projection).
